Action Wellness

Action Wellness enhances the lives of persons with HIV and those at high risk of acquiring HIV by providing a range of trauma-informed social services to help our clients achieve physical, emotional and economic well-being. We offer individualized services to over 2,000 individuals and their families that include: • Medical Case Management • Linkage and Engagement Services After Release from the Philadelphia Department of Prisons (LEAP Program) • Housing Assistance • Buddy Services • Prenatal Support • Immediate Seating Action Wellness staff, volunteers and board members work together to sustain and enhance the quality of life for the diverse communities it serves.
