We empower our clients to make informed decisions throughout their journey with us – from recruiting dedicated and compatible support workers, to helping them integrate within their local community, enabling them to live a fulfilling life, with the right support. Models of care we provide: o Crisis intervention o Fast-track into the community o Discharge to assess o Avoidable admissions o Package breakdowns Bespoke Health and Social Care operate throughout England and hold a CQC registration.
