CCHP is a San Francisco-based health plan. Founded in 1986, our mission is to provide quality, affordable health care coverage to the communities we serve. We offer a full-line of health coverage choices for Employer Groups, Individuals & Families, and people eligible for Medicare Advantage Plans. And, we are one of the few Qualified Health Plan partners to participate in Covered California insurance marketplace. Our coverage area includes City and County of San Francisco and parts of San Mateo County.
