Congregational Home is a non-profit, faith-based Continuing Care Retirement Community (CCRC) offering a Rare Full Continuum of Care for seniors: Short-Term Subacute Rehabilitation, Independent Living, Assisted Living, Memory Care, Skilled Nursing Home Care and Hospice Care – all under one roof. We are one of a small percentage of organizations in the region with this full continuum and the only one in Brookfield.
