Gray Healthcare is a national provider of clinically-informed packages of support to individuals with complex mental health conditions, learning disabilities, autism, acquired brain injuries or other complex needs within a home of their own. Registered with the CQC under the hospital directorate, we help referrers find a lasting community-based solution for the most complex individuals in their catchment areas, individuals who: • Are experiencing a delayed discharge • Have a history of one or more failed placements with other providers. • Are subject to various legal frameworks (e.g. DoLS, CTO, COP etc) • Have forensic histories (including those detained under section 37/41 of the Mental Health Act 1983 (2007) • Have experienced trauma or adversity • Have an ongoing clinical need post discharge • Have developed coping mechanisms of self-harm and suicide ideation • Have a dual diagnosis and/or co-existing chronic physical condition Our framework is clinically informed and embedded with trauma-informed principles. Everything about the support we deliver is tailored to each person, including our specialist staff training programmes, clinical assessments and in-reach service. What’s our property solution? Our in-house property team do all the hard work, such as sourcing suitable housing in the preferred geographical area and managing applications for housing benefit and social housing. Other benefits: • Reassurance that individuals referred to us will have greater independence and improved quality of life as evidenced in our annual clinical outcomes report. • Any cost savings passed back to referrer - 1/3 of the people we support need less support hours now, usually achieved within the first two years. • Property kept separate from support package – no need for an individual to move from their forever home because of a change of provider. • Less time spent sourcing alternative options due to repeated failed community placements.
