Park DuValle Community Health Centers (PDCHC) is a federally qualified health center (fqhc) with 7 locations in Louisville, Taylorsville, and Eminence, KY. PDCHC offers Adult Health, Pediatrics, Women's Health, Dental, and Behavioral Health services. The second floor of the main office also houses an Insurance/Medicaid Enrollment office, WIC enrollment office, and Community Health Workers. Our patients also have access to our in-house lab, x-ray, and 340b discount pharmacy. Transportation to and from is available for those who are in need. PDCHC serves all individuals regardless of income and insurance standing, offering a sliding fee scale as well as accepting medicare, medicaid and most major insurances.
