Plan and Grow is an NDIS and Department of Communities accommodation and support service determined to empower vulnerable children, young people, and adults to achieve their dreams on Whadjuk Noongar Boodjar. Supported Independent Living (SIL), Long and Short Term Accommodation, COSA, respite care, Support Workers, Mentors, Children and Youth Out of Home Care Provider! Our model of care is centred around a trauma-informed approach, remembering to keep the participant at the heart of all decisions made about their care.
